There have been recent major changes to the CEFTRIAXONE SODIUM monograph. Download and print this Ceftriaxone monograph.
There have been recent major changes to the IRON POLYMALTOSE COMPLEX monograph. Download and print this modified Iron Polymaltose Complex monograph. A new monograph for IRON SUCROSE has been added. Download and print this new IRON SUCROSE monograph.The monographs have been changed for the 6th reprint revised (May 2007) so apply to all earlier reprints of the 3rd edition.
Omission from TRASTUZUMAB monograph on page 400. Download and print this modified TRASTUZUMAB monograph and attach to your copy of the AIDH 3rd Edition. This amendment applies only to copies of AIDH 3rd Edition received before September 2005. Copies supplied after this date have been amended before shipping.
The CLINDAMYCIN monograph on page 99 has been amended slightly - tobramycin has been deleted from the list of incompatible drugs. Download and print this revised CLINDAMYCIN monograph. This amendment has been included in the 4th reprint, so only applies to earlier reprints of the 3rd edition.
There have been significant changes to the GENTAMICIN monograph on page 175 relating to the administration of intravenous GENTAMICIN. Download and print this revised GENTAMICIN monograph. This amendment has been included in the 4th reprint, so only applies to earlier reprints of the 3rd edition.
There have been minor changes made to both the POTASSIUM ACETATE and the POTASSIUM PHOSPHATE monographs. Down load and print these revised monographs - potassium acetate, potassium phosphate. These revised monographs have been included in the 4th reprint revised so only apply to earlier reprints of the 3rd edition.
