Medication Management Review accreditation

Registered pharmacists can be accredited to undertake medication management reviews (MMRs) – including Home Medication Reviews (HMRs) and Residential Medication Management Reviews (RMMRs) for individuals in community or aged care settings – which are funded under the 7th Community Pharmacy Agreement (7CPA). Medication management reviews play an important role in reducing re-admissions related to medicine misadventure, and addressing medication-related issues such as side effects. For older patients, and those with complex conditions, medication management reviews can be extremely helpful. 

SHPA is one of only two professional bodies which accredits pharmacists to conduct this cognitive pharmacy service. To become accredited, registered pharmacists must have a combination of professional experience, participation in a continuing professional development program, and evidence they have completed a relevant competency-based assessment such as:

  1. Certification in Pharmacotherapy by the Board of Pharmaceutical Specialties
  2. Certification as a Geriatric Pharmacist by the Board of Pharmaceutical Specialties (formerly CCGP)

Accreditation must be renewed annually, and continued registration and completion of CPD requirements is mandatory to maintain MMR accreditation.
